Health History

We have provided a Health History Questionnaire for you to complete PRIOR TO your first visit with us.  You will discover the process of becoming a new patient quite comprehensive.  It is designed to be thought provoking and requires a commitment of time on your part.  Completed forms will give our practitioners the information that is necessary to begin a treatment plan that is tailored to you!

Consent to Treat

The purpose of obtaining informed consent is to verify that patients have been fully informed of available treatment options including their potential risks. This means that the patient understands each option and can make a decision in accordance with their acceptable quality of life.  Your Acupuncturist will be reviewing all aspects of the treatments with you at your first visit.

Insurance

Our clinic is currently out of network with most insurance companies. This allows us to provide high quality care and not let treatment be dictated by insurance limitations.

Payment

Payment is due at time of service by cash, personal check, or debit/credit card.
